When making a request please use this form:

Type: (avitar/sig/banner)
Text: (what you want it to say)
Text Color:
Background Color:
Character: (name and where the character is from)
Render: (if you have a render or a preferred image that you want us to use, place the url or pic here)
Size: (large or small)
Extras: (claimed, etc.)
Other: (any other ideas that you can think of)
